Symptoms
An upgrade from 11.2.0.2 to 11.2.0.3 leaves the V$_DIAG view and other related ADR views (such as V_$DIAG_HM_RUN, V_$DIAG_VSHOWINCB, etc.) invalid.
This prevents the upgrade from finishing successfully.
NOTE:
The issue may not be limited to above example versions as ig has been reported once also for upgrading 11.2.4.0 32-bit to 11.2.0.4 64-bit on Windows.
