DB node reports predictive failure via ASR with no actual disk failure during exadata image upgrade (Doc ID 1520049.1)

Symptoms

DB node reports predictive failure via ASR with no actual disk failure.  Investigating the event logs of a sundiag does not show any errors or problems with the disks, but does show power-state transitions on and off for the hotspare.

Reviewing the logs closely, this has only been reported to occur during image update to OS image 11.2.3.1.0. The fix for ASR is in 11.2.3.1.0 however the failure occurs during transition of the image updating when the fix is not yet in place.
