RAC Database upgrade using DBUA fails with error ORA-01102 (Doc ID 1545443.1)

Symptoms

 Database upgrade using DBUA fails, the below errors.

[Thread-228] [ 2013-04-11 14:50:27.409 EDT ] [SummarizableStep.genSummary:409] Generating Summary for:=Oracle Server ,status is:=Failed
oracle.sysman.assistants.util.step.StepExecutionException: Failed to start the RAC database exclusively in UPGRADE mode. The following error occured:
ORA-01102: cannot mount database in EXCLUSIVE mode

Upgrade failed due to running the step "Upgrading Oracle Server"

at oracle.sysman.assistants.util.step.StepContext$ModeRunner.run(StepContext.java:2708)
at java.lang.Thread.run(Thread.java:637)

Database alert log file indicates, the remote instance of the database is automatically getting started during the upgrade.

* instance 2 at version 11.2.0.2, rolling upgrade from before 11.2.0.3 <<<
USER (ospid: 10953): terminating the instance due to error 1102


oerr ora 1102
01102, 00000, "cannot mount database in EXCLUSIVE mode"

// *Cause: Some other instance has the database mounted exclusive or shared.
// *Action: Shutdown other instance or mount in a compatible mode.
