RMAN Duplicate from active database fail with ORA-00283 ORA-448 ORA-10878 intermittently

Symptoms

RMAN Duplicate from active database (from standby database) fails during application of archived redologs.

Sometimes subsequent reattempts work fine.
Sometimes a warm restart of the instance corrects the problem.

++ Issue is frequent but intermittent
