Symptoms
When creating a new ASM environment via RMAN, the following errors are generated:
RMAN> BACKUP AS COPY DATABASE FORMAT '<diskgroup>';
RMAN-00571: ===========================================================
RMAN-00569: =============== ERROR MESSAGE STACK FOLLOWS ===============
RMAN-00571: ===========================================================
RMAN-03009: failure of backup command on ORADISK_1 channel at 12/27/2012 12:29:45
ORA-19504: failed to create file "+DATA1"
ORA-17502: ksfdcre:4 Failed to create file +DATA1
ORA-03113: end-of-file on communication channel
The ASM alert log is showing:
Exception [type: SIGSEGV, Address not mapped to object] [lxcsu22m()+16] [flags: 0x0, count: 1]
ORA-07445: exception encountered: core dump [lxcsu22m()+16] [SIGSEGV] [Address not mapped to object] []
