Symptoms
When attempting to mount a diskgroup on one node, the following errors are seen:
NOTE: cache opening disk 0 of grp 4: (already open) <DATA> path:/dev/oracleasm/disks/<disk>
NOTE: F1X0 found on disk 0 au 2 fcn 0.0
NOTE: cache mounting (not first) (retry) external redundancy group 4/0xF0991004 (DATA)
NOTE: LGWR attempting to mount thread 3 for diskgroup 4 (DATA)
NOTE: LGWR found thread 3 open ckpt=7.1036 - signalling ORA-15133
NOTE: LGWR caught ORA-15133 while mounting diskgroup 4
WARNING: instance recovery required during mount
