Symptoms

Trying to restart SQL/Services fails:

@SYS$STARTUP:SQLSRV$STARTUP71
Installing Oracle SQL/Services images
%REGISTER-I-SUMMARY  images examined: 1, dependent images: 0
Starting Oracle SQL/Services
Server started

Oracle SQL/Services failed to start correctly.

Checking the monitor log in SYS$STARTUP, usually called SQS_<node name>_MON_071.LIS, there is an error reported:

%SQLSRV-E-SHMEMEXISTS, Shared memory already exists; still mapped by hung process

 

NOTE: the references above are for SQL/Services 7.1. The number at the end of the filename will change for 7.2, 7.3, etc.
