The ASM instance was upgraded from to following Doc ID: 1434351.1. Step 17 of Doc ID 1434351.1 says that database instances can be added to the restart stack. The standalone database was added to the restart stack following Doc ID 1422517.1, but the database resource still shows OFFLINE:
