ORA-27070 "HPUX-ia64 Error: 15" After Adding Disk To Diskgroup (Doc ID 1604055.1)

Symptoms

While adding disks to a diskgroup errors reported

  ORA-27070: async read/write failed
  HPUX-ia64 Error: 15: Block device required

Diskgroup taken dismounted and the database instances using it is impacted.

 

SQL> alter diskgroup DGDATA112 add disk '/dev/roracle/data23'

NOTE: Assigning number (3,10) to disk (/dev/roracle/data23)
NOTE: requesting all-instance membership refresh for group=3
NOTE: initializing header on grp 3 disk DGDATA112_0010
NOTE: requesting all-instance disk validation for group=3
