Symptoms
Case I. Upgrade Grid Infrastructure from 11.2.0.2 to 11.2.0.3, rootupgrade.sh fails with error: ORA-00600: internal error code, arguments: [ksxp_rm_can_switch4], [196609], [197124], [], [], [], [], [], [], [], [], [] during start ora.asm.
11.2.0.3 <GRID_HOME>/cfgtoollogs/crsconfig/rootcrs_<node>.log shows
