ORA-1691: unable to extend lobsegment when inserting to a LOB table (Doc ID 1612758.1)

Symptoms

The below error is seen in the alert log when inserts are done on a table with securefile.

However no client side ora-1691 or any other error is shown and all rows get inserted successfully.

 ORA-1691: unable to extend lobsegment
