XDB UNINITIALIZED: XDB$SCHEMA NOT ACCESSIBLE in alert log when starting upgrade (Doc ID 1619145.1)

Symptoms

When trying to start the database in 'upgrade' mode, the following error occurs in the alert log

 "XDB UNINITIALIZED: XDB$SCHEMA not accessible
