Adding Cluster Node.. Root.sh Fails With Configuration Of ASM with Error ORA-15031

Symptoms

Adding a node to an 11gR2 cluster. When running root.sh on the new node it fails with:

CRS-2676: Start of 'ora.cssd' on 'mualorat05' succeeded

Disk Group ASMDATADG creation failed with the following message:
ORA-15018: diskgroup cannot be created
ORA-15031: disk specification 'ORCL:DISK4' matches no disks
ORA-15031: disk specification 'ORCL:DISK3' matches no disks
ORA-15031: disk specification 'ORCL:DISK2' matches no disks
ORA-15031: disk specification 'ORCL:DISK1' matches no disks


Configuration of ASM ... failed
see asmca logs at /opt/oracle/cfgtoollogs/asmca for details
Did not succssfully configure and start ASM at /crs11/oracle/product/11.2.0/grid/crs/install/crsconfig_lib.pm line 646

ASMLIB disks are listed on all nodes even then it fails:

Disk "DISK1" is a valid ASM d

Changes

 Customer's SA cloned OS from existing running node.
