Symptoms

A query  fails  with  ORA-07445: exception encountered: core dump [kghalp()+1288] [SIGSEGV] [ADDR:0x3B] [PC:0x1000AE2E8] [Invalid permissions for mapped object] [] 
You may find information about a failure in  kghalp as a  result  of  null current cursor work heap during compile-time in the incident trace file.

Stack trace in the 'Call Stack' section of the incident trace file should containt the .QESMAINITCACHE'. Here is a sample of the critical part of a call stack:
