Instance Keeps Crashing With ORA-00600: [kcbchg1_28] Errors After Windows Patchset 28 Is Installed (Doc ID 1631402.1)

You have installed the Patchset 28 on Windows 64-bit.
After this, the database is attempting to recover from a failed transaction and is terminating the instance with ORA-600 [kcbchg1_28] errors.  Even after restarting the instance, SMON will  continue to terminate the instance with the same errors.

The alertlog shows errors such as:

Errors in file <diagnostic_dest>\diag\rdbms\<dbname\<instname>\trace\<instname>_smon_3928.trc  (incident=9719):
ORA-00600: internal error code, arguments: [kcbchg1_28], [0], [0], [], [], [], [], [], [], [], [], []
PMON (ospid: 1996): terminating the instance due to error 474


The trace file will show a stack trace similar to:
