Symptoms

When the following sqlldr command is executed, the following errors occur:
sqlldr userid=apps/apps control=TEST.ctl  log=TEST.log bad=trace.bad discard=TEST.dsc data=130906122842_test_20130830.csv
appear next error:
SQL*Loader-925: Error while ulmtsyn: OCIStmtExecute(tabhp)
ORA-01115: IO error reading block from file  (block # )
ORA-01110: data file 351: '+DATA/dev/datafile/system.611.820461197'
ORA-15081: failed to submit an I/O operation to a disk

AND


The following trace file is written to the database:

Errors in file /opt/oracle/product/diag/rdbms/dev/DEV/trace/DEV_ora_22676.trc:
ORA-15186: ASMLIB error function = [asm_init],  error = [18446744073709551611], mesg = [Driver not installed]
ERROR: error ORA-15186 caught in ASM I/O path
