Startup of the service fails with PRKP-1030 immediately after adding the service (Doc ID 1640469.1)

Symptoms

Created a new service on the RAC database and startup of the service fails with:

$ srvctl start service -d <dbname> -s <servicename>
PRKP-1030 : Failed to start the service <servicename>.
CRS-0215: Could not start resource 'ora.<dbname>.<servicename>.cs'.

The output generated by export SRVM_TRACE=TRUE shows:

main] [10:4:48:133] [HAStartOperation.run:89]  OUTPUT> Attempting to start `ora.<dbname>.<servicename>.cs` on member `<nodename1>`
[main] [10:4:48:133] [HAStartOperation.run:89]  OUTPUT> `ora.<dbname>.<servicename>.cs` on member '<nodename1>` has experienced an unrecoverable failure.
[main] [10:4:48:133] [HAStartOperation.run:89]  OUTPUT> Human intervention required to resume its availability.
[main] [10:4:48:133] [HAStartOperation.run:95]  ERROR> CRS-0215: Could not start resource 'ora.<dbname>.<servicename>.cs'.
[main] [10:4:48:133] [LocalCommand.execute:56]  LocalCommand.execute: Returned from run method
[main] [10:4:48:134] [HAOperationResult.getOutputAll:114]  outLine is []
[main] [10:4:48:134] [HAOperationResult.getOutputAll:115]  errLine is [CRS-0215: Could not start resource 'ora.<dbname>.<servicename>.cs'.]
[main] [10:4:48:134] [StartAction.executeService:252]  service exception for <servicename>.cs
PRKP-1030 : Failed to start the service <servicename>.
