DBUA:Upgrade of database to 11.2.0.4 fails with “Unexpected error occured.Look at the log file for more details”

Symptoms

 Upgrading single instance database  to 11.2.0.4 in RAC environment fails with following errors.

Below errors are reported in the trace.log:

java.lang.NullPointerException
