ORA-07445 [qesmaIterNext()+40] running query

While running a query, the client may get an ORA-3113 error and the alert log reports and error like:
ORA-07445: exception encountered: core dump [qesmaIterNext()+32]
Exception [type: SIGSEGV, Address not mapped to object] [ADDR:0xFFFFFFFFFDFFFF8] [PC:0x1056F1820, qesmaIterNext()+32] [flags: 0x0, count: 1]
DDE: Problem Key 'ORA 7445 [qesmaIterNext()+32]' was flood controlled (0x2) (incident: 645659)
ORA-07445: exception encountered: core dump [qesmaIterNext()+32] [SIGSEGV] [ADDR:0xFFFFFFFFFDFFFF8] [PC:0x1056F1820] [Address not mapped to object]
