ORA-07445 [kgghstmap()+8] Causes PMON to Terminate Instance (Doc ID 1674716.1)

PMON terminated the instance after an ORA-07445 [kgghstmap()+8]

The alert log reports errors like

Exception [type: SIGSEGV, Address not mapped to object] [ADDR:0x8] [PC:0x2BC2640, kgghstmap()+8] [flags: 0x0, count: 1]
Errors in file <diagnostic_dest>/diag/rdbms/<dbname>/<instname>/trace/<instname>_ckpt_17153.trc  (incident=86657):
ORA-07445: exception encountered: core dump [kgghstmap()+8] [SIGSEGV] [ADDR:0x8] [PC:0x2BC2640] [Address not mapped to object] []
Incident details in:<diagnostic_dest>/diag/rdbms/<dbname>/<instname>/incident/incdir_86657/<instname>_ckpt_17153_i86657.trc
Use ADRCI or Support Workbench to package the incident.
