You encounter an error(s) such as the following:
Exception [type: SIGSEGV, Address not mapped to object] [ADDR:0x7FFFD40DBCD8] [PC:0x93ADCE6, qcpibfa()+180] [flags: 0x0, count: 1]
Errors in file /appl/oracle/diag/rdbms/prime/prod/trace/prod_ora_7557.trc (incident=1183269):
ORA-07445: exception encountered: core dump [qcpibfa()+180] [SIGSEGV] [ADDR:0x7FFFD40DBCD8] [PC:0x93ADCE6] [Address not mapped to object] []
Incident details in: /appl/oracle/diag/rdbms/prime/prod/incident/incdir_1183269/prod_ora_7557_i1183269.trc

You can usually find the failing query in the 'Current SQL Statement' of the incident trace file. Check for an excessive number of parenthesis. For example, queries including the following format could fail;

select empno, ename,job from emp where (((23948 open parenthesis(deptno=10) or (depnto=20) 23948 closed parenthesis )))


This type of query could be created by some Business Intelligence tools.
