Symptoms

 1. Database crashes with the below error:

2. There could be multiple error in the alert log file:

ORA-00600: internal error code, arguments: [4400], [560], [], [], [], [], [], [], [], [], [], []
ORA-00600: internal error code, arguments: [729], [33608], [space leak], [], [], [], [], [], [], [], [], []
ORA-00020: maximum number of processes (2000) exceeded
ORA-20 errors will not be written to the alert log forthe next minute. Please look at trace files to see all the ORA-20 errors.
       Process m000 submission failed with error = 20
ORA-00600: internal error code, arguments: [kghfrmrg:nxt], [0x7000102224A7630], [], [], [], [], [], [], [], [], [], []

3. Review of the generated tracefiles reveals a call stack similar to:

Changes

 Upgraded the database to Oracle 11.2.0.4 version
