Symptoms

ISSUE

 There was an issue with an index identified.
 Customer was advised to rebuild the index.
 During rebuild operation the following error was reported:
 
  SQL> alter index SYS.WRH$_SQL_PLAN_PK rebuild online;
  alter index SYS.WRH$_SQL_PLAN_PK rebuild online
  *
  ERROR at line 1:
  ORA-01452: cannot CREATE UNIQUE INDEX; duplicate keys found
