Oracle DB stops & restarts in windows cluster unexpectedly (Doc ID 179802.1)

Symptoms

You are working with a Windows Cluster and with Oracle Fail Safe.
Fail Safe had passed Verify Cluster and Verify Group without problems.
Sometimes a database will be restarted or the group will be moved to another node. It looks like a database crash, but there is no trace file and nothing appears in alert.log file.
You can see in Windows application event log:

Oracle Fail Safe resource XXX failed to complete IsAlive poll within timeout interval.
