An ORA-07445 [kgghash()], ORA-07445 [kgghtIterInit()] or ORA-07445 [kgghtIterInit2()] error could occur when gathering statistics on a primary/standby database.

The call stack will contain module kzia_ro_fixed_table_callback(), e.g.:
ORA-07445 [kgghash()]
kgghash() <- kgghtAddCB() <- kzia_ro_fixed_table_callback() <- qerfxFetch() <- qerandvFetch() <- qergsFetch() <- opifch2() <- kpoal8() <- opiodr() <- kpoodrc() ...

ORA-07445 [kgghtIterNext()]
kgghtIterNext() <- kzia_ro_fixed_table_callback() <- qerfxFetch() <- rwsfcd() <- qerltFetch() <- ctcdrv() <- opiexe() <- opiosq0() <- kpooprx() ...


 The database has performed a switch over, to or from a read-only standby database, possibly many hours before.
