CLSRSC-214 Error When Executing root.sh and rootupgrade.sh (Doc ID 1910022.1)

Symptoms

[1] Running rootupgrade.sh fails on the node 2 of two nodes cluster for 11.2.0.x to 12.1.0.1 Grid Infrastructure upgrade, rootupgrade.sh shows:

Entries will be added to the /etc/oratab file as needed by
Database Configuration Assistant when a database is created
Finished running generic part of root script.
Now product-specific root actions will be performed.
Using configuration parameter file: <GI_HOME>/crs/install/crsconfig_params
2014/07/22 13:24:12 CLSRSC-363: User ignored prerequisites during installation

2014/07/22 13:27:16 CLSRSC-329: Replacing Clusterware entries in file '/etc/inittab'

2014/07/22 13:27:21 CLSRSC-214: Failed to start 'ohasd'

Failed to start the Clusterware. Last 20 lines of the alert log follow:

 

[2] After GI installation, root.sh fails with similar error (<GRID_HOME>/cfgtoollogs/crsconfig/root_<nodename>_<date>.log) :

2018/09/19 05:35:47 CLSRSC-214: Failed to start the resource 'ohasd'

Failed to start the Clusterware. Last 20 lines of the alert log follow:

2018/09/19 05:35:47 CLSRSC-318: Failed to start Oracle OHASD service

Died at [<GI_HOME>/crs/install/crsinstall.pm line 3041.
