Symptoms

Database crash during Netapp's Takeover and Givebacks operations.

Several I/O errors reported in alert.log until the instance crashes.

Snipped from the alert.log showing the following IO errors errors:

ORA-00345: redo log write error block 557503 count 2

ORA-00312: online log 101 thread 1: '<DIR>/redo_1_101a.rdo'
ORA-27061: waiting for async I/Os failed
Linux-x86_64 Error: 5: Input/output error Additional information: -1 Additional information: 1024


ORA-00202: control file: '<DIR>/control01.ctl'
ORA-27072: File I/O error
Linux-x86_64 Error: 5: Input/output error Additional information: 4 Additional information: 1 Additional information: -1 Errors in file

ORA-00340: IO error processing online log 101 of thread 1
