Inserts Missing from Fast-refresh Materialized View When Direct Path Load Done on Source Table (Doc ID 1915378.1)

Symptoms

 You have a fast-refresh, read-only materialized view.   After batch loads are done on the source table, and the materialized view is fast refreshed, you notice that the inserts are missing (updates and deletes are okay).  The materialized view may be either LOCAL or DISTRIBUTED.  The materialized view is now out of sync with its source table.
