ORA-04030 (callheap,qeeOpt: qeesCreateOpt) - High Memory Usage From A Query Using Inlists

Symptoms
Oracle release 11.2.0.3 version, database instance is having many sessions run huge PGA used size and the overall amount are larger than pga aggregate target .
With internal 200gb of RAM and memory_max_target of 12G, all sessions are consuming greater than 180Gb.
There are a lot of intermittent ORA-04030 errors with arguments (callheap,qeeOpt: qeesCreateOpt).
Analyzing the sessions PGA memory usage shows that the bulk of the memory used here is being allocated as permanent memory in the callheap for chunks
of label "qeeOpt: qeesCre".
The stack for the error is: qeesCreateOpt qerilCopyInList qerilStart qerjotFetch
