Exadata reimaging using USB fails to boot USB (Doc ID 1919001.1)

Symptoms

After using the makeImageMedia.sh script from the ImageMaker kit (from edelivery) to create a USB bootable disk, the USB will not boot up to begin imaging. The system hangs during boot and displays a "grub>" prompt. This issue affects both storage cell and Linux DB node images. This does not affect ISO-based imaging or PXE-based imaging processes. 

This issue is found only with Exadata software versions 12.1.1.1.0 and 12.1.1.1.1.
