ConfigToolAllcommands fails with error-The plug-in Creating Container Database for Oracle Grid Infrastructure Management Repository has failed its perform method (Doc ID 1923941.1)

Symptoms

Upgrading GI to 12.1.0.2, rootupgrade.sh is successfully on all the nodes and OUI screen has been lost, so ConfigToolAllcommands scripts has been run to complete the post rootupgrade tasks as per <note  1360798.1> but it fails with the below error.

dbca is failing during  Mgmt DB creation as per the below permission  issues during configToolAllCommand execution
