After Upgrade to Oracle 12c Some Packages Fail With ORA-07445 [qkaconReferencedCols()+435]

Symptoms

After upgrading to Oracle 12.1.0.1 some user schema packages fail to execute, returning error ORA-3113 to the user.

The alert log reports the following error:

ORA-07445: exception encountered: core dump [qkaconReferencedCols()+435] [ACCESS_VIOLATION] [ADDR:0x0] [PC:0x7FF773E42613] [UNABLE_TO_READ] []


The failing SQL statement in the trace file involves a connect by and a correlated select list subquery, e.g:

SELECT DISTINCT A.PARENT_TAXON_OB , ( SELECT MAX( B.USAGE ) FROM PORTFOLIO.ACCOUNT_TAXONOMY_ITEMS B WHERE B.TAXONOMY_OB = A.TAXONOMY_OB AND B.TAXON_OB = A.PARENT_TAXON_OB ) FROM PORTFOLIO.ACCOUNT_TAXA_PARENTS A WHERE A.TAXONOMY_OB = :B1 START WITH A.TAXON_OB = ANY ( SELECT DISTINCT B.TAXON_OB FROM PORTFOLIO.RETURN_TEMP_TAXA B WHERE B.CONSTRUCTOR_TERM = :B3 AND B.CONSTRUCTOR_OB = :B2 ) CONNECT BY A.TAXON_OB = PRIOR A.PARENT_TAXON_OB
