Unable to mount diskgroup with ora-15042 after asm instance crash during disk expel (Doc ID 1962329.1)

Symptoms

Unable to mount diskgroup with ORA-15042 after the ASM instance crash during disk expel.

1) drop disk normal

alter diskgroup dg01 drop disk DISK1DG1FG1;

2) During that operation, ASM instance crashes
 see ASM instance crash during disk expell. It is in kfdOfflnePriv().

SUCCESS: grp 1 disk DISK1DG1FG1 emptied
NOTE: erasing header (replicated) on grp 1 disk DISK1DG1FG1
NOTE: erasing header on grp 1 disk DISK1DG1FG1
NOTE: process _x000_ln911asm (3705) initiating offline of disk 0.2111465865 DISK1DG1FG1) with mask 0x7e in group 1
NOTE: initiating PST update: grp = 1, dsk = 0/0x7dda6989, mask = 0x6a, op = clear GMON updating disk modes for group 1 at 7 for pid 21, osid 3705
NOTE: group DG01: updated PST location: disk 0001 (PST copy 0)
NOTE: PST update grp = 1 completed successfully
NOTE: killing instance as part of test at kfd.c:9367 USER (ospid: 3705): terminating the instance due to error 15195  Instance terminated by USER, pid = 3705

3) restart the ASM instance, and try to mount the diskgroup, but it cannot be mountable due to ORA-15042.
