Databases Fail With ORA-03115 And ORA-15064 When ASM Disk Or Exadata Griddisk Goes Offline (Doc ID 1970318.1)

Symptoms

- This specific issue happens when running the combination of Grid Infrastructure 12.1.0.2 and RDBMS 11.2.0.4 or lower and a griddisk goes offline.

- In the case of Exadata this can happen when a routine cell disk confinement activity has started on the cell. You can see the timestamps coincide :

 

- Cell alert.log :
