Symptoms
Service crashes after modify command fails:
[oracle@xxxxxx ~]$ srvctl status service -d <DBName> -s <service_name>
Service prod_srv is running on instance(S) <instance_name>
[oracle@xxxxxx ~]$ srvctl modify service -d <DBName> -s <service_name> -n -i <preferred_instance_name> -a <preferred_instance_name>
PRCD-1113 : Failed to modify service <service_name>
PRCS-1011 : Failed to modify server pool <SrvPoolName>
PRCR-1071 : Failed to register or update server pool ora.<DBname_service_name>
CRS-2714: 'ORCL2' is specified multiple times in 'SERVER_NAMES' of 'ora.<DBname_service_name>'
