Fail Safe cannot restart the database which hangs on startup (Doc ID 2008385.1)

Trying to restart database managed by OFS hangs the instance and OFS shows the database as pending.
This happens if you use OFS to take the database offline and then online, move the group to another node, or do a failover.
The 12c database is using AQ as part of the application. Applications are ODP and .NET applications.

The database shuts down with no issues and the attempted restart by OFS hangs as seen in the alert log.

OFS always starts the database restricted, checks if standby or primary to see if it should be mounted or opened.  Then remove the restricted session. The database hangs waiting for EMON to start and it cannot. It is not a complete hang since it can be aborted.

 This worked in previous version of OFS 3.4.2 and database version 11.2.0.x with the same application using AQ.
