DG4MSQL HY000 Connection Fails and Gives Error General Error. Not TCP/IPServerName (Doc ID 2021557.1)

Symptoms

You are using Database Gateway for SQL*Server (DG4MSQL) 11.2.0.4 to connect to a SQL*Server database and the connection fails.
A gateway trace shows the following error -
