Symptoms
Following error is encountered at the time of database start up:
ORA-07445: exception encountered: core dump [nserrbc()+48] [SIGSEGV] [ADDR:0x28] [PC:0x100A29070] [Invalid permissions for mapped object] []
The call stack from the trace file shows functions like:
... nserrbc nserrbd nscall nsgrcOpen nsgrDo nsgrrg_Register kmmlrl ksucln ksbrdp opirip opidrv ...
The problem still exist after restarting clusterware but can apparently be fixed by restarting the operating system.
