opatchauto Hangs While Applying a Patch to DB home (Doc ID 2049111.1)

Symptoms

12.1.0.1.8 opatchauto is able to apply Jul 2015 PSU 21150782 to GI home, but hangs forever when applying to DB home, log shows it was running the following:
                          COMBO OF OJVM COMPONENT 12.1.0.2.4 DB PSU + GI PSU 12.1.0.2.4 (JUL2015) (Patch 21150782)

Successfully executed the command:
echo 'SPOOL OFF' >>
/opt/ORACLE/app/oracle/product/12.1.0.2/db_1/cfgtoollogs/oplan/spfquery.sql

The spfquery.sql has wrong syntax:

SQL> SELECT DECODE(value',' NULL',' 'PFILE'',' 'SPFILE') '\'Init File Type\
FROM sys.v_'\' WHERE name = 'spfile'

ERROR at line 1:
ORA-00907: missing right parenthesis
