Symptoms
A select from V$PWFILE_USERS fails with next errors in the alert log:
ORA-07445: exception encountered: core dump [kghalf()+244] [SIGSEGV] [ADDR:0xFFFFFFFFFFFFFFF0] [PC:0x91925D0] [Address not mapped to object] []
