Unable to Add Disks to Diskgroup after Failed Attempt to Add Them Previously (Doc ID 2064393.1)

Symptoms

We were working with System Admin to get the new disks added to data diskgroup, got the below error when we tried to add the disks

ORA-15032: not all alterations performed
ORA-15075: disk(s) are not visible cluster-wide

SA requested restart of Server for this, and post restart we started facing issue with mounting of data diskgroup for which we created SR "3-11473726651" and followed up to get the issue fixed.

Now when we check the status of the new disks to be added, its showing the status as attached.

Changes

 Tried to add disks to diskgroup that were not visible cluster wide.  This results in the disks getting a header indicating that they are members of the diskgroup.
