Symptoms

In 11.2.0.4, different processes are failing with:

Process startup failed, error stack:
Errors in file /u01/app/oracle/diag/rdbms/<prod_id>/<INSTANCE_ID>/trace/<INSTANCE_ID>_psp0_97995.trc:
ORA-27300: OS system dependent operation:pipe failed with status: 23
ORA-27301: OS failure message: Too many open files in system
ORA-27302: failure occurred at: skgpspawn2
