Symptoms

1. Instance alert log reports error

ORA-27603: Cell storage I/O error, I/O failed on disk o/10.nnn.nnn.5;10.nnn.nnn.6//box/predicate at offset 0 for data length 0
ORA-27626: Exadata error: 212 (Offload library initialization failed)

2. The ps command doesn't show any offload process on cell 10.nnn.nnn.5

ssh 10.nnn.nnn.5

#ps -ef |grep -i flsrv

#

3. Offload alert.log reports error

Failed to read celloffload.ora file

CELLOFLSRV boot failed to start due to the error 1 (The cellinit.ora file contains incorrect parameters.)
Errors in file /opt/oracle/cell/log/diag/asm/cell/SYS_112331_141117/trace/cellofltrc_11617_0.trc  (incident=1):
ORA-00700: soft internal error, arguments: [setupCellParams_2], [1], [The cellinit.ora file contains incorrect parameters.], [], [], [], [], [], [], [], [], []

Offload logs are located under

 /opt/oracle/cell/log/diag/asm/cell/SYS_112331_141117/trace

/opt/oracle/cell/log/diag/asm/cell/SYS_121212_150617/trace
