My Oracle Support Banner DBCA Fails to Start Database (db) Resource With ASM Diskgroup (Doc ID 2107094.1)

Dbca failed to start db resource with asm diskgroup and below trace generated

PRCR-1079 : Failed to start resource ora.orcl.db
CRS-5010: Update of configuration file "/etc/oratab" failed: details at "(:CLSN00011:)" in
CRS-5016: Process "/u01/app/12.1.0/grid/bin/setasmgidwrap" spawned by agent "oraagent" for action "start" failed: details at "(:CLSN00010:)" in "/u01/app/12.1.0/grid/log/rachost1/agent/crsd/oraagent_oracle/oraagent_oracle.log"
CRS-5017: The resource action "ora.orcl.db start" encountered the following error:
ORA-01078: failure in processing system parameters
ORA-01565: error in identifying file '+DATA/orcl/spfileorcl.ora'
ORA-17503: ksfdopn:2 Failed to open file +DATA/orcl/spfileorcl.ora
ORA-12547: TNS:lost contact

CRS-2674: Start of 'ora.orcl.db' on 'rachost1' failed
CRS-2632: There are no more servers to try to place resource 'ora.orcl.db' on that would satisfy its placement policy

ORA-01034: ORACLE not available

With database creation manually, below error seen

ERROR at line 1:
ORA-00200: control file could not be created
ORA-00202: control file: '+DATA'
ORA-17502: ksfdcre:4 Failed to create file +DATA
ORA-15001: diskgroup "DATA" does not exist or is not mounted
ORA-12547: TNS:lost contact

The disk group is already mounted in ASM.

This issue also hit when upgrading the database from to with ASM storage or while restore a database on ASM storage.
