GI root.sh fails to create ASM instance due to ORA-01017 error (Doc ID 2109710.1)

Symptoms

GI root.sh fails, from root script log:

> ASM failed to start. Check /u01/app/grid/cfgtoollogs/asmca/asmca-160217AM062559.log for details.

Manually tried to logon to ASM instance as a grid user ==>

$ sqlplus / as sysasm

ERROR:
ORA-01017: invalid username/password; logon denied
