Symptoms
Insert statement fails with errors like below in the alert log:
ORA-00603: ORACLE server session terminated by fatal error
ORA-00600: internal error code, arguments: [ktuchg bad xcb], [0x1B3D110B08], [13], [], [], [], [], [], [], [], [], []
