Symptoms
While using DBCA to create a new RAC database the following error is seen on the command line:
PRCR-1154 : Failed to create file output stream with file name: $ORACLE_BASE/cfgtoollogs/dbca/trace.log_DBHome_2016-07-25_08-37-22-AM
Couldn't get lock for $ORACLE_BASE/cfgtoollogs/dbca/trace.log_DBHome_2016-07-25_08-37-22-AM
Connection to <host_name> closed.
Couldn't get lock for $ORACLE_BASE/cfgtoollogs/dbca/trace.log_DBHome_2016-07-25_08-37-22-AM
Connection to <host_name> closed.
Also, a window for Database Configuration Assistant pops up with the following error:
ORA-48189: OS Command to create directory failed
