Symptoms
On : 11.2.0.4 version, SQL*Net with Database Patch Set Update 11.2.0.4.160119
on HP OpenVMS Itanium 8.4
Frequent ORA-3113 process termination when connected to database
Even the most basic commands like creating a tablespace or selecting some v$view can lead to this.
It happens within seconds or a minute after being connected to the database via SQLNet.

The issue can be reproduced at will with the following steps:
1. sqlnet connection to remote instance
2. Issue a command and then repeat the command
