Cannot Create Database On Windows Server 2012 (Doc ID 2197524.1)

DBCA is failing to add a database when used to 'create database' in a Grid Infrastructure Standalone (Oracle Restart) environment.

Specifically the errors PRCD-1025 and PRCT-1011 are encountered, along with the following error output collected from turning on SRVM tracing:


Issuing 'srvctl.bat, -V, -inner, 1] env = [ORACLE_HOME=D:\oracle\database\121, LANG=en_US.UTF-8' from command line SUCCEEDS.
