rootupgrade.sh succeeded on all nodes but activeversion wasn't upgraded due to parallel root script execution (Doc ID 2205144.1)

Symptoms

Upgrading 4-node 11.2.0.4 to 12.1.0.2, rootupgrade.sh succeeded on all nodes:

Node1:

Installer gives the following warning:

INS-41815 The installer has detected that Oracle Clusterware is not upgraded to 12.1.0.2.0 version.

Are you sure you want to continue?

GI activeversion wasn't set to the new version:

# crsctl query crs activeversion
Oracle Clusterware active version on the cluster is [11.2.0.4.0]
